Application Description

After you complete the Interest Form, expect an e-mail with an application and further instructions within 1-2 business days from rromero@LNESC.org.

  • Schedule of availability to participate in program​

COVID-19 SELF-CERTIFICATION FORM

This portion indicates if you or any member of your household have been affected by COVID-19 in the following ways:

  • Loss of income

  • Unemployed

  • Teleworking/Working from home

  • Conducting virtuallearning/attending school online

  • Lack of health insurance

  • Emotional/Loss of Social Connection

  • Loss of internet access

PROJECT SUMMARY

Overview of program services and benefits as a program participant and expectations.

Digital signatures are required by the applicant upon submission of the application.

* The Family Educational Rights and Privacy Act (FERPA), 20 U.S.C. 1232g, and the Texas Public Information Act, Texas Government Code 552.001 et seq., are respectively federal and state laws providing for the review and disclosure of student education records. The University and Outreach Programs- TRIO will not permit access to or the release of personally identifiable information contained in student educational records to any party without the written consent of the student, except as authorized by FERPA.

Interview

After you have submitted a complete application, you will be invited to complete a pre-assessment survey at the beginning of the program for progress tracking services and the development of personalized instruction and curriculum.

In order to complete your acceptance and enrollment into the Digital Navigators Program, all participants must agree and commit to the following:

  • Provide proof of income (one of the following: tax return, W-2s, etc...)

  • Provide proof of address (one of the following: lease agreement, utility bill, or other mail in your name)
